§ 5.04.030. License—Application.

  • Applications for licenses required by this chapter shall be made to the mayor in writing, signed by the applicant if an individual, or by a duly authorized agent thereof if a club, verified by oath or affidavit, and shall contain the following information and statements:

    A.

    The name, age and address of the applicant in the case of an individual; in the case of a partnership, the persons entitled to a share in the profits thereof; and in the case of a club or corporation, the date of incorporation or organization, the objects for which it was organized and the names and addresses of the officers and directors;

    B.

    The citizenship of the applicant and, if an individual, his place of birth; and, if a naturalized citizen, the time and place of his naturalization;

    C.

    The character of business of the applicant;

    D.

    The length of time the applicant has been in business of that character;

    E.

    The amount of goods, wares and merchandise on hand at the time of application;

    F.

    The location and description of the premises or place of business which is to be operated under the license;

    G.

    A statement whether the applicant has made similar application for a similar license on premises other than described in this application, and the disposition of such application;

    H.

    A statement that the applicant has never been convicted of a felony and is not disqualified to receive a license by reason of any matter or thing contained in this chapter, laws of the state or other ordinances of the city;

    I.

    Whether a previous license by any state or subdivision thereof, or by the federal government, has been revoked and the reason therefor;

    J.

    A statement that the applicant will not violate any of the laws of the state or the United States of America, or any ordinance of the city in the conduct of his place of business;

    K.

    Three character references;

    L.

    A statement that the applicant has never been convicted of violating any state or federal law concerning the manufacture, possession of or sale of alcoholic liquor, nor has ever forfeited his bond to appear in court to answer charges for any such violation;

    M.

    The name and address of the owner of the premises described in the application; and, if the premises are not owned by applicant, the term of the lease shall be set forth;

    N.

    A copy of the applicant's dram shop and liability insurance policy in force or issued for one year shall accompany the application for liquor license;

    O.

    A statement that the applicant will not have more than one person in his establishment at any time after closing hours for cleaning. If there is to be more than one person during special cleanup, or other special conditions, the management shall notify the police department to that effect.
